Types of medical documentation

Requests may involve school or camp forms, employment-related documentation, insurance reports and disability forms. Driver’s medical examinations and specialized assessments should be confirmed separately. Not every form can be completed during a brief walk-in visit.

A doctor can document findings and provide a professional opinion within the information available. Approval by an employer, insurer or government program is determined by that organization, not guaranteed by the clinic.

Preparing for a forms appointment

Bring the complete form, instructions, deadline, identification and relevant medical records. Complete patient sections beforehand where appropriate. Some requests need an examination, record review, testing or an appointment with your regular physician.

Can a note be backdated?

Will the form be ready the same day?
